StoneGate Senior Living in Lewisville, TX is seeking a MDS Resource Traveler (RN) or (LVN) to join our team. This role focuses on Medicaid and Medicare reimbursement, coordinating and completing MDS 3.0 assessments and care plans, and timely transmission as required by federal and state regulations.

You will support the interdisciplinary team, ensure regulatory compliance, and contribute to a positive patient experience.

Qualifications

• Current RN or LVN license in the state of practice.
• 3 to 5 years of experience in skilled nursing facility.
• Experience with MDS and Care Plans.
• Proficiency in the RAI / PPS processes.
• Strong attention to detail and documentation.
• Good communication and computer skills.

Responsabilités

• Maintain MDS/Care plan schedule for interdisciplinary team.
• Coordinate preparation of MDS and Care Plan as per regulatory requirements.
• Track Medicare resident benefit days and validate daily Medicare census, coordinating with management for accurate billing.
• Prepare responses to requests for additional information from the intermediary and appeals decisions within established timeframes.
• Prepare Medicare denial letters for each resident discharged from Medicare coverage within Medicare guidelines time limits.
• Participate in ongoing evaluation of documentation required for Medicaid reimbursement.

You will be responsible for the coordination and completion of clinical assessments, supporting documentation, care planning, and transmitting MDS 3.0, all as required by federal and state regulations. You will participate in and direct the delivery of patient care in accordance with facility clinical systems, regulatory requirements, and physician orders. MDS Resource Traveler Responsibilities:

• Maintaining MDS/Care plan schedule for interdisciplinary team
• Coordinating preparation of MDS and Care Plan as per regulatory requirements
• Tracking Medicare resident benefit days, validating daily Medicare census, and coordinating this information with management to assure accurate billing

MDS Resource Traveler Requirements:

• Current RN or LVNlicense in the state of practice
• 3 to 5 years of experience in skilled nursing facility with previous experience in completing MDS’s and Care Plans as an MDS Coordinator preferred
• Demonstrated proficiency in the RAI / PPS processes
• Strong attention to detail, documentation, and resident assessment skills
• Good communication, clerical, organizational, and computer skills
