Legacy Healthcare in Overland Park, KS is seeking an organized MDS Nurse / MDS Coordinator to lead the MDS/RAI process and care planning. You will collaborate with nursing, therapy, dietary, social services, physicians, residents, and families to capture accurate information for person-centered plans.

The ideal candidate has strong knowledge of MDS 3.0, RAI processes, and skilled nursing regulations, with a focus on documentation accuracy and timely assessments.

Qualifications

• RN or LPN/LVN license required by state regulations.
• MDS experience preferred, with SNF/LTC exposure valued.
• Strong knowledge of MDS 3.0 and RAI processes.
• Proven ability to prepare accurate, care-planned documentation.

Responsabilités

• Coordinate the MDS/RAI process and timely assessments.
• Review clinical documentation for accuracy and consistency.
• Develop and coordinate comprehensive person-centered care plans.
• Engage interdisciplinary teams in the assessment and planning process.
• Support survey readiness and regulatory compliance.
