Outfield Healthcare Partners is seeking a licensed Registered Nurse to oversee MDS/RAI processes and ensure accurate care planning for residents in the facility. You will coordinate interdisciplinary teams, monitor assessments, and maintain state and federal compliance through timely submissions.

Responsibilities include leading daily stand-ups, reviewing resident care data, and ensuring proper documentation in MDS programs.

Qualifications

• Licensed RN in New Mexico.
• Extensive knowledge of MDS 3.0 and RAI processes.
• Strong understanding of OBRA regulations and data reporting.
• Experience with Case-Mix and Medicare/Medicaid reimbursement.
• Familiarity with care planning.

Responsabilités

• Coordinate and monitor MDS assessments and RAI processes.
• Lead daily stand-up with interdisciplinary team and set ARD dates.
• Ensure compliance with federal and state regulations for MDS completion.
• Maintain MDS status and accurate data entry in MDS systems.
• Prepare care planning conferences and communicate outcomes.
• Report trends from audits to QA committee.
• Ensure timely submission of MDS to state databases and follow-up on validation errors.
• Assist DON with significant changes and physician orders.
• Participate in regulatory surveys and address deficiencies.
