Legacy Healthcare is seeking a knowledgeable MDS Nurse to join our Skilled Nursing Facility team in a role coordinating the MDS/RAI process. The ideal candidate is an RN or LPN/LVN with strong clinical experience who understands documentation and care planning for long-term care residents.

The MDS Nurse will collaborate with the DON, ADON, nursing staff, therapy, social services, residents, and families to ensure accurate assessments and person-centered care plans, while maintaining compliance

Competenze

• Current RN or LPN license in good standing.
• MDS experience preferred.
• Skilled nursing/long-term care or post-acute experience preferred.
• Knowledge of MDS 3.0 and the RAI process.
• Strong understanding of clinical documentation and care planning.
• Excellent attention to detail.

Mansioni

• Coordinate the MDS assessment process for assigned residents.
• Complete MDS assessments accurately and within required timeframes.
• Review resident charts and clinical documentation for accuracy and completeness.
• Gather information from nursing and all applicable interdisciplinary departments.
• Coordinate interdisciplinary input for MDS assessments.
• Identify missing, incomplete, or inconsistent documentation and follow up appropriately.
• Develop and maintain comprehensive, person-centered care plans.
• Ensure care plans reflect each resident's current condition, needs, goals, and preferences.
• Participate in care plan meetings and interdisciplinary team meetings.
• Monitor residents for significant changes in condition.
• Communicate changes in resident status to appropriate clinical team members.
• Collaborate with the DON, ADON, Unit Managers, nurses, and other care team members.
• Review documentation for consistency between the resident's clinical condition, MDS, and plan of care.
• Assist with quality assurance and performance improvement initiatives.
• Participate in clinical audits and chart reviews.
• Support survey readiness and regulatory compliance.
• Maintain accurate MDS and resident records.
• Stay current with MDS, RAI, and skilled nursing requirements.
• Communicate professionally with residents and families.
• Maintain confidentiality of resident information.
• Assist with identifying opportunities to improve resident outcomes.
• Perform other MDS and clinical duties as assigned.
