Job summary

Pikes Peak Post Acute is seeking an experienced nurse administrator to oversee the MDS process, PDPM initiatives, and care planning for residents. You will ensure timely, compliant assessments, coordinate interdisciplinary teams, and drive data-driven improvements in patient outcomes.

The role requires BSN preferred, licensure in good standing, and 3+ years of supervisory experience, preferably in long-term care. Strong Microsoft Office skills and a commitment to quality are essential.

Qualifications

• Graduation from an Accredited School of Nursing; BSN preferred.
• Must maintain good standing with State Board of Nursing and licensure requirements.
• 3 years supervisory experience preferred; long-term care experience preferred.

Responsibilities

• Oversee MDS assessments to ensure timely and accurate completion.
• Coordinate interdisciplinary participation in MDS and care planning.
• Provide PDPM and MDS training to new coordinators and staff.
• Monitor utilization of PDPM tools and prepare data-driven recommendations.
• Maintain confidentiality of resident care information as required by law.
