Job summary

The Woodlands Healthcare Center in Leesville, LA is seeking an MDS Nurse to oversee timely and accurate MDS processing from admission to discharge, aligning with policies and state regulations.

The role requires active LA RN/LPN licensure and at least 2 years of MDS experience in a nursing facility, with familiarity in rehabilitative/restorative nursing and managed care guidelines.

Qualifications

• Must possess an active LA RN or LPN license.
• Two years MDS experience in a nursing facility preferred.
• Training in rehabilitative/restorative nursing practices.
• Working knowledge of managed care rules, regulations, and guidelines.

Responsibilities

Responsibilities

Responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge and admission tracking, etc. With direction from the VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department.

Qualifications

• Must possess a current, unencumbered, active license to practice as an LPN or RN in LA state.
• Two (2) years’ MDS experience in a nursing care facility preferred.
• Must have training in rehabilitative and restorative nursing practices.
• Must have a working knowledge of managed care rules, regulations, and guidelines.
• Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
