Job summary

Brookhaven Nursing and Rehabilitation Center in Carrollton, TX is seeking a licensed RN or LPN to oversee RAI/MDS processes from admission to discharge, ensuring compliance with federal/state guidelines.

Three years of MDS experience in a nursing facility are preferred. The role involves coordinating clinical information systems and staff education to support quality patient care.

Responsibilities

Responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge and admission tracking, etc. With direction from the VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department.

JOB QUALIFICATIONS

• Must possess a current, unencumbered, active license to practice as an LPN or RN in TX state.


• Three (3) years' MDS experience in a nursing care facility preferred.


• Must have training in rehabilitative and restorative nursing practices.


• Must have a working knowledge of managed care rules, regulations, and guidelines.
