Job summary

Priority Management Group, LLC is seeking an LPN/LVN or RN to manage MDS processes from admission to discharge in accordance with policy and regulatory guidelines. The role requires experience with MDS and care coordination within a nursing facility.

The ideal candidate will hold an active state license and have training in rehabilitative nursing, with a solid understanding of managed care rules and the ability to implement programs that ensure high-quality patient care.

Qualifications

• Must possess a current, active license to practice as an LPN/LVN or RN in this state.
• Three years’ MDS experience in a nursing care facility preferred.
• Training in rehabilitative and restorative nursing practices.
• Working knowledge of managed care rules, regulations, and guidelines.
• Ability to plan, organize, develop, implement, and interpret programs, goals, policies and procedures.

Responsibilities

• Responsible for timely and accurate completion of the MDS process and care management from admission to discharge.
• Coordinate information systems operations and education for the clinical department under VP guidance.

Responsibilities

Responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge and admission tracking, etc. With direction from the VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department.
