The LPN MDS Nurse at Life Care Centers of America is responsible to complete and submit accurate MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations and in accordance with all applicable laws, regulations, and Life Care standards.

This full-time position requires an active LPN/LVN license, at least two years of nursing experience (geriatric preferred), CPR certification, and the ability to work in a post-acute care setting with attention to compliance, confidentiality,

Formación

• Attended an accredited LPN program.
• Currently licensed/registered as an LPN/LVN in the applicable state.
• Two years nursing experience; geriatric experience preferred.
• CPR certification on hire or during orientation; must remain current.

Responsabilidades

• Complete and submit accurate MDS Assessments, CCAs, and Care Plans.
• Report changes in a patient’s condition identified by the MDS.
• Provide education to direct care staff regarding CMS/RIA updates.
• Assist in reviews of Interdisciplinary Care Plans and Final Validation Reports.
• Participate in required meetings and maintain professional conduct.
