Job summary

Life Care Center of the South Shore is seeking an experienced MDS Nurse to join our skilled nursing and rehabilitation team. Full and/or part time hours considered for the right candidate.

The LPN MDS Nurse will complete and submit accurate MDS assessments, CCAs, and care plans according to CMS RAI manual regulations and ensure compliance with all applicable laws and Life Care standards.

Qualifications

• Attended an accredited LPN program.
• Current LPN/LVN license in good standing.
• Two years of nursing experience; geriatric preferred.

Responsibilities

• Complete and submit MDS assessments, CCAs, and care plans per CMS RAI manual.
• Report changes in patient condition to the Director of Nursing.
• Educate direct care staff on CMS/RAC updates affecting documentation.
• Contribute to interdisciplinary care planning and reviews.
• Monitor and ensure timely submission and billing readiness.
• Demonstrate professional conduct and maintain patient safety.
