Job summary

Life Care Centers of America is seeking an LPN MDS Nurse in Hendersonville, NC. You will complete MDS assessments, CCAs, and care plans in line with CMS RAI guidelines and Life Care standards.

Requirements include an accredited LPN program, active LPN license in good standing, and two years of nursing experience with geriatric care preferred. CPR certification is required and must stay current.

Qualifications

Responsibilities

• Complete and submit MDS Assessments, CCAs, and Care Plans per CMS RAI.
• Report changes in patient condition to the DON.
• Educate direct care staff on CMS RAI updates impacting documentation.
• Assist with review of the Interdisciplinary Comprehensive Care Plan.
• Review CMS Reports and ensure timely billing of assessments.
• Participate in PPS Meetings and required cross-team meetings.
• Perform as a staff nurse when needed.
• Maintain patient rights, safety, and sanitation.
• Demonstrate reliability and teamwork.
