Priority Management Group, LLC in Carrollton, TX seeks an LPN or RN with MDS experience to oversee the MDS process and discharge planning for residents. You will ensure care coordination from admission through discharge in compliance with federal and state guidelines.

Qualifications

• Current, active TX license as LPN or RN.
• Three (3) years’ MDS experience in a nursing facility preferred.
• Training in rehabilitative and restorative nursing practices.
• Working knowledge of managed care rules, regulations, and guidelines.
• Ability to plan, organize, develop, implement, and interpret programs, goals and procedures for quality care.

Responsibilities

• Responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge and admission tracking, etc. With direction from the VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department.

JOB QUALIFICATIONS

• Must possess a current, unencumbered, active license to practice as an LPN or RN in TX state.
• Three (3) years’ MDS experience in a nursing care facility preferred.
