Job summary

Life Care Centers of America is seeking an LPN MDS Nurse to complete and submit accurate MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI Manual regulations.

The role requires an active LPN license, two years of nursing experience (geriatrics preferred), and CPR certification. The position emphasizes independent judgment, compliance, and strong teamwork within a post-acute care setting.

Qualifications

• Attended an accredited LPN program.
• Active LPN/LVN license in good standing.
• Two years of nursing experience; geriatric experience preferred.
• CPR certification on hire or during orientation.

Responsibilities

• Complete and submit MDS Assessments, CCAs, and Care Plans per CMS RAI Manual.
• Notify DON of changes in patient condition identified by MDS Assessment.
• Educate direct care staff on CMS RAI Manual updates affecting documentation.
• Assist with review of Interdisciplinary Comprehensive Care Plan.
• Review final validation reports and ensure CMS QIES system readiness before billing.
• Identify assessments completed or submitted late and help develop prevention processes.
• Participate in PPS meetings and other required team meetings as requested.
• Provide care as a staff nurse when needed and maintain excellent patient service.
