Job summary

Life Care Centers of America seeks an LPN MDS Nurse in Orlando to accurately complete MDS Assessments, CCAs, and Care Plans per CMS RAI manual and state regulations. The role requires an active LPN/LVN license and minimum two years of nursing experience, preferably geriatric, with CPR certification on hire.

You will work in a post-acute care setting, ensure timely documentation, participate in daily meetings, and maintain compliance with company policies and confidentiality standards.

Qualifications

• Must be licensed as LPN/LVN in the applicable state.
• Two years of nursing experience; geriatric care preferred.
• CPR certification required on hire.
• Adheres to CMS RAI and facility policies.

Responsibilities

• Complete and submit accurate MDS Assessments, CCAs, and Care Plans.
• Report changes in a patient’s condition to the DON.
• Educate direct care staff on CMS RAI updates and documentation.
• Review and validate MSDS/Final Validation Reports and CMS submissions.
• Attend PPS meetings and other required meetings; participate in team.
• Perform staff nurse functions as needed; maintain patient rights and safety.
