Job summary

Cambridge Rehabilitation and Nursing Center in Cambridge, MA is seeking RN or LPN candidates for full-time, part-time, and per diem roles to join our long-term care team. Qualifications

• Graduate of an accredited school of nursing for RN or LPN licensure.
• One year experience in long-term care; knowledge of MDS and Care Plan process.
• Familiarity with PPS and PDPM concepts.

Responsibilities

• Schedules and completes MDS assessments and care plans for residents.
• Monitors MDS completion by disciplines; ensure CMS/RAI guidelines and RN sign-off.
• Advises supervisor of incomplete assessments by other disciplines.
• Ensures timely accuracy of MDS/CAAs/Care plans sheets.
• Initiates care plans and activities for best outcomes.
• Generates monthly care plan calendar for following month.
• Conducts care plan conferences for residents.
• Maintains and updates care plans quarterly and adjusts as needed.
• Reviews incidents and 24-hour reports to reflect changing needs.
• Facilitates interdisciplinary care conference coordination.
• Monitors 7-day look-back documentation within MDS timeframes.
• Participates in facility management meetings as requested.
• Audits clinical documentation for accuracy and timeliness.
• Advises on persistent documentation issues to Nursing leadership.
• Promotes ongoing education to stay current with regulations.
• Maintains knowledge of RAI Manual and MDS PDPM processes.
• Attends in-service training and education programs.
• Protects resident confidentiality and rights in all duties.
• Supports quality of care and attendance standards.
• Performs duties safely per guidelines and expectations.
• Reports unsafe conditions and incidents promptly to supervisors.
• Promotes company standards for customer service with internal/external customers.
• Complies with policies and codes of conduct.
