Willowbrook is seeking a Nursing Supervisor to oversee the facility’s PDPM/MDS workflow and resident assessment process. You will coordinate interdisciplinary teams, ensure timely MDS submissions, and guide new MDS coordinators through training and compliance.

The role emphasizes accurate documentation, regulatory adherence, and ongoing quality improvement in long-term care. Strong leadership and state licensure are essential.

Formación

• BSN preferred, State Board of Nursing in good standing.
• 3 years supervisory experience preferred.
• Long Term Care experience preferred.

Responsabilidades

• Oversee facility weekly PDPM meetings for skilled patients.
• Monitor PDPM tools usage and data quality (MDS, CSR, weekly tracking).
• Provide PDPM and MDS training to new coordinators.
• Assess health status and level of care for new admissions.
• Ensure timely completion of all MDS assessments including PPS Medicare and annual reviews.
• Coordinate interdisciplinary participation in MDS for new admissions.
• Maintain accurate schedule and data entry for MDS submissions.
• Ensure confidentiality of resident care information.
