Kadima Healthcare Group is seeking an MDS Nurse - RN/LPN to join our team and move into a supervisory role. The role centers on directing resident assessments (MDS) and coordinating with the DON and care team to ensure high-quality resident care in line with Pennsylvania regulations.

The ideal candidate has RN or LPN licensure, a nursing degree, and prior experience in skilled nursing settings, with an emphasis on MDS processes and care planning.

Qualifications

• Graduate of an accredited Nursing program required.
• Licensure and registration as RN or LPN required.
• Prior experience in skilled nursing facility preferred.
• Previous MDS experience highly preferred.

Responsabilités

• Conduct and coordinate resident assessments (MDS) per state regulations.
• Evaluate resident condition and data to inform assessments.
• Develop and implement care plans with the DON and team.
• Schedule assessment and care plan meetings with the care team.
• Assist in coordinating participation by resident representatives and families.
• Ensure care aligns with resident wishes and discharge planning.
• Ensure complete resident assessments are documented and signed.
