Responsibilities

• Coordinate resident assessments per PA rules and facility policies.
• Develop and implement care plans with the team to meet resident needs.
• Assist DON and staff in scheduling assessments and care plan meetings.
• Ensure care plans reflect resident wishes and measurable objectives.
• Transmit resident information to the state as required.
