Job summary

Kadima Healthcare Group is seeking an MDS Nurse - RN/LPN to oversee the resident assessment process and coordinate comprehensive care plans in a skilled nursing setting in Pennsylvania. This position offers progression toward supervisory responsibilities.

Responsibilities include conducting assessments, coordinating care-plan meetings with the care team and family, ensuring care aligns with resident wishes, and supporting discharge planning.

Qualifications

• Graduate of an accredited Nursing program required.
• Licensure and registration by the PA Board of Nursing Examiners as RN or LPN required.
• Previous MDS experience highly preferred.

Responsibilities

• Conduct and coordinate the resident assessment (MDS) in accordance with Pennsylvania requirements and facility policies.
• Develop and implement the care plan with the DON and inform all assessment team members of new admissions.
• Schedule and participate in resident assessment/care plan meetings with the care team and family representatives.
• Ensure care provided aligns with the resident's wishes and support discharge planning.
• Coordinate the written plan of care, including problems, goals, and responsible professionals.
• Ensure timely completion of initial assessment (within 14 days) and comprehensive care plan (within 21 days).
• Assist nursing staff and other departments to ensure adherence to the care plan and regular reviews.
