Job summary

Kadima Healthcare Group is seeking an MDS Nurse - RN/LPN to join our care team in Pennsylvania. This role focuses on directing and coordinating resident assessments (MDS) in accordance with state requirements and facility policies.

You will supervise nursing staff, collaborate with the DON and care teams, and ensure assessments and care plans are accurate, timely, and person-centered for residents' physical, emotional, and spiritual needs.

Qualifications

• Graduate of an accredited Nursing program required.
• Licensure and PA nursing registration as RN or LPN required.
• Prior experience in a skilled nursing facility, acute care setting, or related field preferred.
• Previous MDS experience highly preferred.

Responsibilities

• Interview or coordinate the interview of each resident for the assessment.
• Evaluate resident condition and data to determine need for assessment updates.
• Develop and implement procedures with the DON for assessment team notifications.
• Assist scheduling of resident assessment/care plan meetings.
• Coordinate participation by care plan team members and family representatives.
• Ensure care aligns with resident wishes and discharge planning.
• Develop the plan of care with measurable objectives and timelines.

Education

Graduate of an accredited Nursing program

RN or LPN licensed/registered by PA Board of Nursing Examiners
