Kadima Healthcare Group is seeking an MDS Nurse - RN/LPN to join our Pennsylvania team. This role offers a clear path into supervisory responsibilities for the right candidate.

The MDS Nurse will conduct and coordinate resident assessments per state requirements and facility policies, ensuring timely completion and collaboration with the care planning team to meet residents' needs and goals.

Qualifications

• Graduate of an accredited Nursing program required.
• Licensure and registration by the PA Board of Nursing Examiners as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) required.
• Prior experience in a skilled nursing care facility, acute care setting, or other related field preferred.
• Previous MDS experience highly preferred.

Responsibilities

• Interview residents for the assessment.
• Evaluate resident's condition and medical data to determine the need for special assessment activities or plan amendments.
• Develop and implement procedure with the DON to inform all assessment team members of the arrival of newly admitted resident.
• Assist facility directors and supervisors in scheduling the resident assessment/care plan meetings.
• Coordinate participation by outside members of the care plan team, including the resident's representative and/or family members.
• Ensure that care provided is in accordance with the resident's wishes.
• Assist with the care plan portion of the resident's discharge plan.
• Develop preliminary and comprehensive assessment of nursing needs using required forms.
• Coordinate development of written plan of care identifying problems, goals, and responsible professionals.
• Ensure care plans include measurable objectives and timetables for identified needs.
• Ensure initial and comprehensive assessments are completed within mandated timeframes.
