Job summary

Kadima Healthcare Group is seeking an MDS Nurse - RN/LPN to join our Pennsylvania team in a supervisory role. The position centers on coordinating and completing resident assessments in line with Pennsylvania requirements and facility goals.

The role involves leading the assessment team, developing care plans, and ensuring timely, accurate MDS submissions while supporting residents and their families throughout the process.

Qualifications

• Must be licensed as RN or LPN in Pennsylvania.
• Graduate from an accredited nursing program.
• MDS knowledge and assessment experience preferred.

Responsibilities

• Interview residents for MDS assessments and coordinate the process.
• Evaluate resident conditions and data to determine needs for assessments.
• Develop and implement procedures with the DON to inform the assessment team.
• Assist in scheduling resident assessment and care plan meetings.
• Coordinate participation of care plan team members including family representatives.
• Ensure care aligns with resident wishes.
• Assist with care plan discharge planning.
• Coordinate development of written care plans identifying problems, goals, and services.
• Ensure care plans have measurable objectives and timelines.
• Ensure assessments are completed within required timeframes.
• Oversee revision of care plans after quarterly reviews.
• Inform team of accuracy requirements for the MDS.
• Ensure all portions of the assessment are signed and dated.
• Transmit resident information to state agencies as required.
