Job summary

Caraday Healthcare is seeking an MDS-LVN to support the Director of Nursing in ensuring documentation meets federal, state and certification guidelines. The role coordinates the RAI process, ensuring timeliness and completeness of MDS, CAAS, and the Interdisciplinary Care Plan.

Responsibilities include participating in surveys, interfacing with staff, monitoring PPS timelines, and maintaining accurate documentation for Medicare/Medicaid and other payers.

Qualifications

• Current LVN license
• CPR certification within 30 days of hire
• Thorough knowledge of RAI process, Medicare PPS, and Medicaid reimbursement

Responsibilities

• Assist with regulatory surveys and address deficiencies
• Coordinate RAI process from MDS through care plan
• Monitor and report trends from audits to QA Committee
• Ensure completion of MDS, care plans, and signatures
• Manage RAI process tracking and PPS/Medicaid forms
• Support staff to maintain documentation accuracy and compliance
