Job summary

Forest Park Nursing & Rehabilitation in Missouri is seeking an experienced MDS Coordinator to support a fully staffed facility. The role ensures timely completion of the RAI and care management processes from admission to discharge in line with policy, guidelines, and federal/state requirements.

Under direction from the Director of Nursing and VP of Clinical Reimbursement, you may coordinate information systems operations and education for the clinical department.

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments.
• Decide on whether to complete additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings to review resident care and assessment dates.
• Ensure compliance with federal and state regulations for MDS processes.
• Monitor MDS and care plan documentation to support coding in medical records.
• Maintain current MDS status per state and federal guidelines.
• Enter resident data into MDS software accurately.
• Complete MDS coding via chart review, observation, and interviews.
