Job summary

Forest Park Nursing Center is seeking an experienced MDS Coordinator to oversee timely and accurate completion of MDS and care management from admission to discharge, following company policy and federal/state guidelines.

Under direction from the Director of Nursing and VP of Clinical Reimbursement, you will coordinate information systems operations for the clinical department and ensure proper documentation in the medical record. At least 2 years in a Skilled Nursing Facility is preferred.

Qualifications

• RN or LVN licensed in the state of practice is required.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding of OBRA regulations and Minimum Data Set (MDS).
• Experience with MDS 3.0 is preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to schedule MDS assessments and care plan reviews.
• Decide on whether to complete additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings and review resident care and assessment references.
• Ensure compliance with federal/state regulations for MDS processes.
• Monitor and document MDS and care plans in the medical record.
