Job summary

Forest Park Nursing Center is seeking an experienced MDS Coordinator to manage MDS/Care Plan processes from admission to discharge in compliance with state and federal guidelines. The role requires RN or LVN licensure and at least 2 years of SNF nursing experience.

You will coordinate interdisciplinary MDS scheduling, ensure accurate coding, and supervise data entry across MDS programs, under Dir. of Nursing and VP of Clinical Reimbursement.

Qualifications

• RN or LVN licensed in state of practice.
• Minimum 2 years nursing experience in Skilled Nursing Facility.
• Thorough knowledge of Case-Mix, Medicare PPS and Medicaid reimbursement.
• Familiarity with OBRA regulations and MDS 3.0 preferred.
• Proficient in the care planning process.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide with team whether to complete additional MDS assessments based on clinical criteria.
• Assist with daily stand-up meetings to review resident care and assessment reference dates.
• Comply with federal and state regulations for MDS completion and coordination.
• Monitor MDS and care plan documentation to ensure proper coding in the medical record.
• Maintain current MDS status for assigned residents per guidelines.
• Ensure timely data entry of resident information into MDS systems.
• Accurately code MDS using chart Review, observations, and staff interviews.
• Other duties may be assigned as needed.
