Rocky River Gardens in Ohio is seeking an experienced RN/LPN MDS Coordinator to coordinate the RAI process, ensuring timely and complete MDS, CAA's, and interdisciplinary care plans. Responsibilities include monitoring assessments, inputting MDS data, transmitting to the state and obtaining validation receipts, scheduling care plan conferences, and leading monthly QM meetings.

Qualifications

• Must hold a Nursing Degree from an accredited college or university.
• Active RN/LPN license and CPR certification.
• At least 2 years’ experience in Long Term Care as an MDS Coordinator.

Responsibilities

• Monitors and guides the completion of assessments in a timely manner.
• Inputting MDS assessments and transmission to the state; obtain validation receipt.
• Schedules and facilitates care plan conferences and QM meetings.
• Manages Care Planning Process.
