Job summary

West Village Post Acute in Greenville, SC seeks an experienced MDS Coordinator (RN) to ensure accurate and timely MDS assessments in compliance with state, federal, and Medicare guidelines.

The role requires active SC RN licensure and prior MDS experience, with strong PDPM and regulatory knowledge. The position is full-time, on-site, with opportunities for growth and cross-team collaboration at a 132-bed facility.

Qualifications

• Active RN license in South Carolina (in good standing).
• Prior MDS experience required.
• Strong knowledge of PDPM, Medicare/Medicaid, and long-term care regulations.
• Experience in skilled nursing facility (SNF) setting preferred.
• Excellent attention to detail and organizational skills.
• Strong communication and teamwork abilities.

Responsibilities

• Complete and submit MDS assessments accurately and on time.
• Ensure compliance with CMS, Medicare, and Medicaid regulations.
• Coordinate with the interdisciplinary team for care plan development.
• Review and monitor documentation to support reimbursement accuracy (PDPM).
• Participate in care plan meetings and QA processes.
• Track and manage assessment schedules and deadlines.
• Educate staff on documentation and regulatory requirements.
• Collaborate with nursing, therapy, and administration teams.
