West Village Post Acute in Greenville, SC seeks an experienced MDS Coordinator (RN) to ensure accurate and timely MDS assessments in compliance with state, federal, and Medicare guidelines. This full-time role supports the interdisciplinary team to optimize care plans and reimbursement.

You will manage schedules, educate staff on documentation, and coordinate with nursing, therapy, and administration to uphold quality and regulatory standards. Competitive salary and benefits offered.

Qualifications

• Active SC RN license in good standing.
• Prior MDS experience.
• Strong knowledge of PDPM, Medicare/Medicaid, and long-term care regulations.
• Experience in SNF setting preferred.
• Excellent attention to detail and organizational skills.
• Strong communication and teamwork abilities.

Responsabilités

• Complete and submit MDS assessments accurately and on time.
• Ensure compliance with CMS, Medicare, and Medicaid regulations.
• Coordinate with the interdisciplinary team for care plan development.
• Review and monitor documentation to support reimbursement accuracy (PDPM).
• Participate in care plan meetings and QA processes.
• Track and manage assessment schedules and deadlines.
• Educate staff on documentation and regulatory requirements.
• Collaborate with nursing, therapy, and administration teams.
