Chatham Hills Subacute Care Center is seeking an experienced MDS Coordinator to join our clinical leadership team. The role focuses on accurate MDS assessments, regulatory compliance, and collaboration with interdisciplinary teams to optimize resident care and reimbursement.

The ideal candidate holds an active New Jersey RN or LPN license and has MDS 3.0, RAI, and PDPM knowledge. Strong organizational and communication abilities are essential for meeting deadlines in a demanding skilled nursing

Formación

• Active New Jersey RN or LPN license.
• Previous MDS experience in skilled nursing facilities.
• Strong knowledge of MDS 3.0, RAI guidelines, PDPM, and CMS regulations.
• Experience coordinating interdisciplinary care plans.
• Excellent organizational and time-management skills.

Responsabilidades

• Coordinate the facility's MDS assessment process per CMS guidelines and regulatory requirements.
• Ensure timely and accurate completion and submission of MDS assessments.
• Collaborate with nursing, rehabilitation, social services, dietary, and other interdisciplinary team members.
• Lead and coordinate interdisciplinary care plan meetings.
• Monitor assessment schedules, care plans, and documentation for accuracy and completeness.
• Ensure proper use of the RAI Manual, MDS 3.0, and PDPM guidelines.
• Identify opportunities to improve clinical outcomes, documentation, and reimbursement accuracy.
• Maintain compliance with federal and state regulations and participate in audits and surveys.
• Provide guidance and education to staff regarding MDS documentation and assessment requirements.
