Job summary

West Village Post Acute is seeking an experienced MDS Coordinator (RN) in Greenville, SC for a full-time role with an annual salary of $85,000–$100,000. The facility emphasizes quality care, regulatory compliance, and interdisciplinary collaboration to optimize MDS processes and reimbursement.

The candidate will ensure timely MDS assessments, coordinate with care teams, and maintain compliance with CMS/Medi caid guidelines in a SNF setting.

Qualifications

• Active RN license in South Carolina (in good standing).
• Prior MDS experience required.
• Strong knowledge of PDPM, Medicare/Medicaid, and long-term care regulations.
• Experience in SNF setting preferred.
• Excellent attention to detail and organizational skills.
• Strong communication and teamwork abilities.

Responsibilities

• Complete and submit MDS assessments accurately and on time.
• Ensure compliance with CMS, Medicare, and Medicaid regulations.
• Coordinate with the interdisciplinary team for care plan development.
• Review and monitor documentation to support reimbursement accuracy (PDPM).
• Participate in care plan meetings and QA processes.
• Track and manage assessment schedules and deadlines.
• Educate staff on documentation and regulatory requirements.
• Collaborate with nursing, therapy, and administration teams.
