Job summary

Outfiled Healthcare is seeking an experienced MDS Coordinator for our facility. The role involves ensuring timely completion of the RAI process and care management from admission to discharge, under policy and regulatory guidelines.

Collaborate with the interdisciplinary team, monitor MDS status, and maintain accurate documentation to support coding and reimbursement. Relocation bonus is available for full-time hires.

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Excellent knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Thorough understanding of OBRA regulations and the MDS process.
• Knowledge of the care planning process.
• Experience with MDS 3.0 is preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Ensure decisions to complete or not complete MDS assessments follow the RAI Manual.
• Assist with coordinating daily stand up meetings and updating assessment reference dates.
• Comply with federal and state regulations for the MDS/RAI process.
• Monitor MDS and care plan documentation for accuracy in medical records.
• Maintain current MDS status per state and federal guidelines.
• Maintain data entry for resident information in MDS software.
• Complete accurate MDS coding from chart review and interviews.
• Other duties as assigned.
