Job summary

Signature HealthCARE is seeking an MDS-focused RN/LPN to provide coverage for open positions, ensuring accurate resident assessments across our long-term care facilities. This role requires travel Monday through Friday, with overnight stays as needed, to support MDS processes and program implementation.

The ideal candidate holds RN or LPN licensure and has at least 3 years of MDS experience in long-term care, with strong knowledge of RAI, PPS, and Medicare guidelines.

Qualifications

• Current licensure as a Registered Nurse or Licensed Practical Nurse.
• Minimum of three (3) years MDS experience in a long-term care setting.
• Travel to centers up to 100% of the time with overnight stays as needed.

Responsibilities

• Provide MDS coverage for open positions, educate and implement programs that apply to the Resident Assessment Instrument (RAI), Prospective Payment Systems (PPS), Quality Measures (QM’s), Medicaid Casemix and Medicare guidelines for all SHC stakeholders.
• Assist and collaborate with Clinical Reimbursement Specialist with onboarding of new MDSC.
• Exhibit competency in all areas of the RAI, Care Planning, PPS, Medicare, and when applicable Medicaid Case Mix
