Job summary

Signature HealthCARE is seeking an experienced MDS Leader to oversee Resident Assessment Instrument (RAI) processes, PPS and QM measures, and Medicare/Medicaid guidelines across multiple facilities. You will train stakeholders, audit MDS processes, and ensure high-quality care with autonomy and accountability.

The role requires valid RN licensure (multi-state preferred) and at least five years in long-term care as an RN, with travel Monday–Friday across assigned sites.

Qualifications

• MDS experience in LTC settings preferred.
• Willing to travel 80–90% with overnight stays.
• Detail-oriented with strong organizational skills.
• Ability to train stakeholders and coordinate with sites.

Responsibilities

• Support systems development and implementation for RAI, PPS, QM, and Medicare/Medicaid guidelines across SHC sites.
• Design and present RAI, PPS, QM materials utilizing diverse media.
• Conduct site visits to orient new MDS Coordinators and reinforce processes.
