Job summary

Signature HealthCARE is seeking a motivated MDS Leader to join our team. The ideal candidate is a Registered Nurse with multiple years of long-term care experience and a proven ability to lead MDS coordination across sites.

The role requires frequent multi-site travel (80%–90%) with overnight stays. You will train stakeholders and guide RAI/PPS processes, enhancing quality outcomes and resident-centered care across our network.

Qualifications

• Current licensure as a Registered Nurse in good standing.
• MDS experience in long-term care within the last two years.
• Ability to travel 80–90% with overnight stays.

Responsibilities

• Assist with systems development and implementation of RAI, PPS, QM, Medicaid Casemix and Medicare/Medicaid guidelines for SHC Stakeholders.
• Design and present RAI, PPS, QM, and Medicare education using various media, workshops, and one-on-one sessions.
• Conduct facility site visits to orient new MDS Coordinators and reinforce RAI/MDS/PPS processes.
