Job summary

Trilogy Health Services, LLC in Indianapolis, IN, seeks an MDS Coordinator (LPN or RN) to oversee resident assessments and care planning, ensuring compliance with federal and state regulations and Medicare/Medicaid reimbursement.

You will conduct MDS assessments, develop care plans, and communicate with residents and families about Medicare/Medicaid options, while maintaining accurate MDS notes and supporting care teams at Clearvista Lake Health Campus.

Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• 3–5 years’ experience working in the MDS or assessment role in a senior residential care, healthcare, senior living industry or long-term care environment, preferred.
• Current, valid CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment to evaluate residents’ physical, psychological and functional status, including Care Area Assessments (CAA) and triggers.
• Evaluate each resident’s condition to determine any need for special assessment activities or amendments to the admission assessment.
• Prepare and electronically transmit timely reports to the national Medicare and Medicaid databases.
• Develop a written plan of care for each resident identifying problems/needs and goals.
• Provide information to residents/families on Medicare/Medicaid and other financial assistance programs.
• Ensure MDS notes are informative and descriptive of services and resident responses.
• Assist with completing the care plan portion of the discharge plan and participate in committees as they pertain to assessment and/or care plan functions.
