Job summary

Woodmont Health Campus in Valparaiso, IN, is seeking an MDS Coordinator (LPN, RN) responsible for overseeing the resident assessment and care planning process. This role involves conducting MDS assessments to evaluate residents’ condition and needs, ensuring compliance with regulations, and preparing reports.

Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• Three to five years’ experience working in the MDS or assessment role is preferred.
• Current, valid CPR certification required.

Responsibilities

• Conduct and complete the Minimum Data Set (MDS) assessment.
• Evaluate each resident’s condition and medical data.
• Develop a written plan of care for each resident.
• Provide information on Medicare/Medicaid and financial assistance programs.
• Ensure MDS notes are informative and descriptive.
• Assist with completing the care plan for discharge.

Skills

Current, valid state LPN or RN license
