Job summary

Wurtland Nursing and Rehab is seeking an Experienced MDS Coordinator, LPN/RN to coordinate MDS and care plans for residents in our Kentucky facility. The role requires an active RN or LPN license and familiarity with LTC medical records and MDS 3.0 processes.

The MDS Coordinator will supervise all MDS activities, ensure timely submissions, and lead care plan meetings in compliance with state and federal regulations. Strong communication and computer skills are essential.

Qualifications

• Must hold current RN/LPN nursing license in the state of employment.
• Experience in Long Term Care Medical Records and MDS processes.
• Proficient in Data Entry and Word processing; familiarity with MDS 3.0.

Responsibilities

• Coordinate and supervise MDS and Care Plans for residents in a timely manner.
• Inform care team when care plans are due and ensure MDS portions are completed.
• Develop or revise resident care plans quarterly or with condition changes.
• Manage PPS and state MDS assessments; ensure MDS, CATs and CAAs are completed.
• Schedule and lead care plan meetings; encourage team participation.
• Submit MDS to state/federal agencies per Medicare/Medicaid guidelines.
• Maintain dates for MDS/CAAS completion; safeguard resident rights.
