Job summary

CMR Recruiting in Baltimore, MD is seeking an experienced MDS Coordinator (RN or LPN) for our skilled nursing facility. This is a full-time, on-site role with competitive pay and comprehensive benefits.

The MDS Coordinator owns the resident assessment process from admission to discharge, transmits MDS on schedule, and leads care plan meetings with the interdisciplinary team. Qualifications include Maryland RN or LPN license, knowledge of MDS 3.0 and PDPM, and strong documentation skills; on-site

Qualifications

• Active Maryland RN or LPN license
• Working knowledge of the MDS 3.0 assessment process and PDPM
• Experience in a skilled nursing or long term care setting preferred
• Strong documentation and computer skills
• Able to work full time on site in the building

Responsibilities

• Complete, code and transmit MDS assessments within CMS timeframes
• Schedule and track the assessment calendar for all residents
• Lead care plan meetings with nursing, therapy, dietary and social services
• Review documentation for accuracy and reimbursement support
• Serve as the building resource for resident assessment questions
