You will ensure compliance with Medicare/Medicaid guidelines, collaborate across disciplines, and contribute to achieving exceptional resident care outcomes. A current RN license is required.

Qualifications

• Must hold an active RN license in the state of employment.
• Working knowledge of the MDS 3.0 (current version).
• Preferred: professional trained Registered Nurse.

Responsibilities

• Conduct and coordinate MDS assessments, develop individualized care plans, and ensure timely updates for changes in residents’ conditions.
• Ensure accurate submission of MDS assessments per Medicare/Medicaid guidelines and state/federal regulations.
• Facilitate care plan meetings and support cross-department communication to improve resident care outcomes.
