Job summary

Wellbrooke of Wabash, part of Trilogy Health Services, seeks an MDS Coordinator (LPN/RN) to oversee resident assessments and care planning, ensuring regulatory compliance for Medicare/Medicaid reimbursement.

Responsibilities include conducting MDS assessments, planning care, communicating with families about financial assistance, and coordinating discharge plan activities within a long-term care setting.

Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• Three (3) to five (5) years' experience working in the MDS or assessment role in a senior residential care, healthcare, senior living industry or long-term care environment, preferred.
• Current, valid CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment to evaluate residents' physical, psychological and functional status, including CAAs and triggers.
• Evaluate each resident's condition and medical data to determine need for special assessment activities or amendment of admission assessment.
• Prepare and electronically transmit timely reports to the national Medicare/Medicaid databases.
• Develop a written plan of care for each resident identifying problems, needs and goals.
• Provide information to residents/families on Medicare/Medicaid and other financial assistance programs available.
• Ensure MDS notes are informative and descriptive of services provided and resident response.
• Assist with completing the care plan portion of the discharge plan and implement recommendations from committees.
