Job summary

East Cobb Center for Nursing and Healing seeks an experienced MDS Coordinator (RN or LPN) to oversee resident assessments in line with CMS and LTCI guidelines. You will review MDS data, coordinate the RAI/MDS workflow, and collaborate with nursing and interdisciplinary teams to optimize care plans.

You will generate QM/QI reports, support quality initiatives, and ensure timely communication across disciplines to maintain high-quality resident care.

Qualifications

• Must be an RN or LPN with knowledge of MDS/RAI processes.
• Familiarity with CMS and LTCI regulations and guidelines.
• Ability to collaborate with nursing staff and other disciplines.

Responsibilities

• Review and process MDS assessments per CMS/LTCI requirements.
• Collaborate with nursing caregivers and multidisciplinary teams to align care plans with MDS data.
• Prepare accurate QM/QI reports based on RAI/MDS data.
• Ensure timely completion and transmission of RAI/MDS across departments.
