Job summary

Roswell Center for Nursing and Healing in Roswell, GA, is hiring an MDS Nurse (RN or LPN) to coordinate Resident Assessment Instrument/MDS processes and ensure CMS/LTCI compliance. The role involves leading interdisciplinary care plan meetings, updating plans, and driving quality improvements.

You will collaborate with nursing staff and other disciplines to ensure accurate RAI/MDS submissions and provide educational support to the team as needed.

Qualifications

• Must be an RN or LPN.

Responsibilities

• Initiates and actively participates in Interdisciplinary Care Plan meetings.
• Coordinates the development and revision of resident Interdisciplinary Care Plans based on information identified in the RAI/MDS.
• Ensures the Interdisciplinary Care Plan addresses all triggered indicators.
• Collaborates with nursing caregivers and other disciplines to obtain information on residents' status and progress toward care plan goals.
• Demonstrates the ability to communicate effectively with a variety of other disciplines to ensure the accurate and timely completion of the RAI/MDS for transmission.
• Prepares accurate reports of QM/QI derived from the RAI/MDS which drives data for the CLC Compare; tracks and trends data to improve quality of care.
• Active involvement in Quality Improvement.
• Implements an educational plan to meet changing program or service needs for self and others.
