Empire Care Centers' East Cobb Center for Nursing and Healing is seeking an experienced MDS Coordinator (RAI/MDS) to ensure accurate resident assessments and high-quality care. You will review and process MDS, coordinate with CMS/LTCI guidelines, and serve as the nursing lead for documentation.

Must be an RN or LPN. You will collaborate with nursing staff and other disciplines, prepare QM/QI reports, and help drive care planning in a supportive team environment.

• Must be an RN or LPN.

• Effective communication with various disciplines to facilitate timely completion and transmission of RAI/MDS

MDS Coordinator Summary:

As the MDS Coordinator, you'll serve as the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) Coordinator for our center. Your responsibilities will include reviewing and processing MDS assessments in accordance with CMS and LTCI regulations and guidelines. Must be an RN or LPN
