Job summary

Fulton Center for Rehabilitation in Atlanta, GA is hiring an MDS Coordinator (RN or LPN) to lead and manage the RAI/MDS process, ensuring accuracy, compliance, and quality-driven outcomes across the center.

Qualifications

• Must be a Licensed RN or LPN in the State of Georgia.

Responsibilities

• Lead all MDS/RAI processes and documentation.
• Initiate and lead Interdisciplinary Care Plan meetings.
• Coordinate and update resident care plans based on MDS triggers and assessments.
• Collaborate with nursing staff and other disciplines to gather accurate resident data.
• Ensure timely completion and transmission of MDS assessments per CMS guidelines.
• Analyze QM/QI from MDS reports and support performance improvement initiatives.
• Provide training to clinical team members and participate in audits.
