Avid Healthcare in the Quad Cities seeks a dedicated MDS Coordinator to lead and complete the MDS/RAI processes and care plans. This full-time, on-site role requires accurate data gathering, interdisciplinary collaboration, and compliance with regulatory requirements.

You will coordinate assessments, review documentation for accuracy, educate staff on standards, participate in care conferences, and contribute to quality improvement initiatives, ensuring optimal resident care and reimbursement.

Formación

• Current RN or LPN license in good standing.
• Experience with MDS processes and RAI guidelines.
• Experience in care planning and interdisciplinary coordination.
• Geriatric or long-term care nursing experience.
• Strong attention to detail and time management.
• Knowledge of long-term care regulations and reimbursement.
• Excellent communication and collaboration with residents, families, and staff.
• Ability to adapt to regulatory changes and pursue ongoing training.

Responsabilidades

• Coordinate and complete the MDS/RAI processes and care plans.
