Job summary

Allure of the Quad Cities in Moline, Illinois, is seeking a dedicated MDS Coordinator (RN or LPN) to join our care team. You will coordinate resident assessments, develop individualized care plans, and ensure accurate documentation and regulatory compliance.

You will collaborate with nursing, therapy, dietary, social services, and other departments to deliver comprehensive resident care, facilitate family communication, and support ongoing quality improvement.

Qualifications

• Illinois RN or LPN license in good standing.
• Minimum of 1 year of long-term care experience preferred.
• Previous MDS experience preferred.
• BLS certification required.
• Strong organizational, communication, and time-management skills.
• Ability to work collaboratively with an interdisciplinary team.

Responsibilities

• Complete accurate and timely MDS assessments in accordance with regulatory requirements.
• Coordinate and develop individualized care plans that reflect residents' needs, preferences, and goals.
• Work closely with nursing, therapy, dietary, social services, and other departments to ensure comprehensive resident care.
• Facilitate communication among residents, families, and the interdisciplinary team.
• Maintain accurate records and ensure compliance with applicable state and federal regulations.
• Participate in quality assurance initiatives to improve resident outcomes and care delivery.
• Provide guidance and education to staff regarding MDS processes, care planning, and documentation requirements.
