Allure of the Quad Cities in Moline, Illinois, is seeking a dedicated MDS Coordinator (RN or LPN) to join our team. You will coordinate resident assessments, develop individualized care plans, and ensure accurate documentation and regulatory compliance.

Qualifications

• Current Illinois RN or LPN license in good standing.
• Minimum of 1 year of long-term care experience preferred.
• Previous MDS experience preferred.
• Current BLS certification required.
• Strong organizational, communication, and time-management skills.
• Ability to work collaboratively with an interdisciplinary team.

Responsibilities

• MDS assessments: complete accurate and timely MDS assessments in accordance with regulatory requirements.
• Care planning: coordinate and develop individualized care plans reflecting residents' needs and goals.
• Interdisciplinary collaboration: work with nursing, therapy, dietary, social services, and other departments for comprehensive care.
• Communication: facilitate communication among residents, families, and the interdisciplinary team.
• Documentation & compliance: maintain accurate records and ensure regulatory compliance.
• Quality improvement: participate in quality assurance initiatives to improve resident outcomes.
• Education & support: provide guidance and education to staff regarding MDS processes.
