Job summary

Allure of the Quad Cities in Moline, Illinois is seeking a dedicated MDS Coordinator (RN or LPN) to coordinate resident assessments, develop individualized care plans, and ensure accurate documentation and regulatory compliance.

The role collaborates with nursing, therapy, dietary, social services, and other departments to support high-quality care; requires current IL RN or LPN license, BLS, and 1 year long-term care experience; provides benefits including insurance, 401(k), and PTO.

Qualifications

• Current Illinois RN or LPN license in good standing.
• Minimum of 1 year of long-term care experience preferred.
• Previous MDS experience preferred.
• Current BLS certification required.

Responsibilities

• MDS Assessments: Complete accurate and timely MDS assessments in accordance with regulatory requirements.
• Care Planning: Coordinate and develop individualized care plans that reflect each resident's needs, preferences, and goals.
• Interdisciplinary Collaboration: Work closely with nursing, therapy, dietary, social services, and other departments to ensure comprehensive resident care.
• Communication: Facilitate communication among residents, families, and the interdisciplinary team.
• Documentation & Compliance: Maintain accurate records and ensure compliance with applicable state and federal regulations.
• Quality Improvement: Participate in quality assurance initiatives to improve resident outcomes and care delivery.
• Education & Support: Provide guidance and education to staff regarding MDS processes, care planning, and documentation requirements.
