Care Initiatives is seeking an MDS Coordinator (RN) in Iowa to lead comprehensive MDS assessments and coordinate resident care plans. You will work with interdisciplinary teams to ensure regulatory compliance and accurate documentation, supporting quality measures and reimbursement.

The role requires an active Iowa RN license and CPR certification, with a focus on empathy, discretion, and teamwork in a skilled nursing environment. On-site position with potential on-call responsibilities.

Qualifications

• Valid RN license in good standing (Iowa).
• Current CPR certification.
• Knowledge of federal and state long-term care regulations.
• Strong empathy, respect, patience and discretion.
• Willingness to learn and grow with a quality-focused clinical team.

Responsibilities

• Complete and coordinate MDS assessments to evaluate resident needs.
• Collaborate with interdisciplinary teams to update care plans.
• Ensure compliance with CMS/state/federal long-term care regulations.
• Monitor clinical documentation for reimbursement and quality measures.
• Analyze resident data to support clinical decisions.
• Serve as resource on MDS processes and documentation standards.
• Participate in care conferences and communicate with residents/families.
• Support audit readiness and survey-ready documentation.
• Enforce policies and guide team growth.
• Communicate care needs to residents and families.
• Document care to meet standards.
• Follow infection control and safety protocols.
• Participate in on-call rotation with leadership team.
